For Referring Providers

You may refer patients to an AnMed provider:

Include the patient's name, date of birth, address, phone number and insurance information. Attach any relevant medical records, including radiology and lab tests.

After we receive the referral, we’ll call your patient to schedule an appointment. You’ll receive a copy of the consult or office note once the patient is seen.

Third-Party Verification

Use the verification portal torequest verification of a provider's membership and privileges on AnMed's medical staff.
